The following information is required for revoking parking lot access. Please fill out all sections completely. Revocation Forms not filled out properly may cause a delay in processing, which could result parking fees if tenant is over their allotted number of parking space.

Submit completed form to the Property Management Office (phone (301) 493- 5585) located in 6700, A-Wing, Cellar Level. Additionally, the associated Parking Permit and Garage Access Badge must be returned with the form. If these items are not returned, the registered individual or tenant will be charged $20 per item missing.
